Washington State Collaborative to Improve Health

The Washington State Collaborative to Improve Health helps busy practices improve their systems of care - for prevention, screening and early detection of disease – not only for the individual patient, but for all patients who either have or are at risk of developing a chronic disease. We know that chronic diseases are the leading cause of disability and death in Washington and the United States. We also know that we can do a better job of preventing chronic disease in our communities and in treating patients who already live with chronic disease.

The next stage of the Collaborative is designed for adult and pediatric practices with five or fewer physicians. The number of teams will be limited to 10 for each focus area. Small practices are invited to choose to focus on:

*Registration will begin in November 2007 should funding be secured for the pediatric component of the Collaborative. Funding for the adult component has already been secured.

The collaborative will begin in 2008 and will consist of 3 Seattle-area learning sessions over the course of 18 months. The first learning session for the Collaborative will be held May 15, 2008, in the conference center at the Seattle-Tacoma International Airport. The second learning session is scheduled for Nov. 5, 2008. The Collaborative ends with an outcomes congress on May 5, 2009.
